Before your visit
What to bring.
It is fine if you do not have everything. Bring what you can, and the team will help with the rest.
- Photo ID and insurance card
- A list of current medications and supplements, with doses
- Recent PSA or other blood test results
- Imaging reports, and the images themselves if you have them (MRI, CT, ultrasound)
- Biopsy and pathology reports
- Notes or letters from your other doctors
- Your questions, written down
- A family member or friend, if you would like support

Can I send my medical details through this website?

That is what the first visit is for. You do not need to know the name of a procedure to book. Bring your records, and the options will be explained to you.
